newbie clutch question

whats up guys, just had what amounts i'm sure to a stupid question, but i've only worked on automatics and i am currently in the process of converting my 66 coupe (6cyl) to the T5. Anyway, how in the world does the pressure plate, flywheel, clutch disc, etc. fit together with the clutch fork in the bellhousing? My manual desintigrated, and i've searched and searched with no luck for a schematic showing this info. Also, any tricks/tips i should kno before putting it together? Oh and i had my flywheel resurfaced, should i have it balanced as well? Thanks so much everybody, this forum has helped me tremendously over the past few years.

Oh yea, its a 66 200 coupe, V8 T5, 8" rear, 8.5" clutch/flywheel. Thanks again
