yes, it usually comes with the oil pump when you buy a new one

I've never seen one included!
Every one that I've ever purchased always had the pump/pickup tube gasket, but never the pump/block gasket.

I always just make my own. I use the thinnest gasket material I can find.
Do NOT use cork!

I usually put a thin bead of RTV down....REAL THIN. basiclly paint it on as a sealer between the two

Oh, yeah I was thinking of that one.

Never seen one. I don't think that any RTV will hold up with the pressures involved either.
