Yes, the pilot bushing you use will need to match the transmission. The snout of the C4 torque converter rode directly in the crankshaft with no bushing as the torque converter did not spin in relation to the crankshaft. The 3-speed manual transmission will likely be a 2.77, so you will need a pilot bushing for a 200 with that transmission (a 170/2.77 bushing might fit too, but since they put 2.77 trans behind 200's in 65 and 66 in Mustangs that's what I'd get).
 
If the van has a small6 in it already, everything you need should already be there. You will need flywheel etc as the automatic doesn't use an actual flywheel. The crank itself is the same.
