Installed new Clay Smith 264 cam with 108°center lobe in my 66 200. Everything I read indicates I need to advance 4°. So does that mean if I verify that the cam is 108° center lobe with the timing marks aligned TDC and I then adjust the timing sprocket + 4° advance? On the next dial in, should I see 104° or 112°?

I don't no what YOU mean (clarity is explaing one'n only one way ! ):
all ways degree your cam. Never go dot to dot w/o a ck 1st...
 
For 4 degrees advanced the intake lobe center needs to be at 104 degrees ATDC.
If you look at your cam card the .050" duration is 214 degrees and the intake opening .050" lobe lift will be at 3 degrees BTDC and the closing .050" lobe lift will be at 31 degrees ABDC.
These are the points that you should be using on your degree wheel and reading with use of your dial indicator.
