Rocker Slip

What causes or has anyone seen a rocker arm slip off the valve? Started the mustang the other day and heard a helluva rasping sound. Pulled the valve cover and found the rocker arm pushing on the valve spring on cylinder 5 intake. Just wandering if this happens to anyone else and any way to prevent it?
 
There are springs between them to hold them in place.
Perhaps one is failing or damaged. Maybe from over-revving?
The shaft could be worn.
 
Yea the spring between the intake and exhaust was was compressed b/c as soon as i loosened my rocker assembly it kicked back over. Just wandering if there was any way to prevent that?
 
8) what some racers have done with the FE engines, they use a similar system as the small six, is to replace the springs with spacers to eliminate the problem altogether. if you choose to do this, you will have to make your own spacers as none exist to my knowledge.
